Home Health RN

 Due to increasing demand, Lincoln Park Manor is seeking to expand our current Skilled Home Health services team.

Full-Time position available.  Hours are Monday-Friday 8:00am to 4:30pm.  Alternating paid on-call responsibility.

No home visits on holidays or weekends.

 Duties include establishing care programs in coordination with the client and the client’s physician. Meeting all documentation and visit requirements per Medicare Skilled Regulations. Ensuring accurate information is shared with client, family, physician and therapy services.  Utilizing clinical expertise to increase referral base.  Necessary training will be provided.

Job Qualifications:

Candidate needs to be a motivated self-starter, possessing good communication skills.  Someone who believes in a positive, holistic approach to home health care and can work in an environment with an open heart and an open mind.  Must possess current RN license in the State of Kansas. Valid driver’s license and reliable insured transportation.
