Solomon Valley Hospice Registered Nurse

Mitchell County Hospital Health Systems is looking for a full-time Registered Nurse (40 hours per week) Monday-Friday 8:00 am- 4:30 pm for our Hospice department. The proper candidate will be responsible for identifying and coordinating care for terminally ill patients and their families in homes, skilled nursing facilities or residential care facilities. Job duties include admitting new patients, completing admission/initial assessment as well as associated documentation, so excellent computer documentation skills as well as communication skills is a must. Frequency of patient/family contacts is determined by the patients plan of care. As the patients case manager, the nurse is also responsible for leadership and oversight of the entire interdisciplinary plan of care. The RN will endeavor to utilize teaching, assessment and intervention skills to provide quality end of life for patients and their families. This job includes taking rotating night, weekend, and holiday call (approximately 8 nights per month). The qualified applicant must have a minimum of 2 years recent RN clinical experience. Hospice and or Home Health experience is preferred.
