First Care Clinic is seeking applications for the position of Patient Care Coordinator.  Duties for this position include, but are not limited to:

• Providing chronic care management services.
• Educating/counseling patients on preventative and chronic health care issues.
• Assisting providers in clinic as needed.
• Triaging patients.
• Assisting in patient group education classes.
• Completing nurse visits including immunizations and lab draws.

Applicants for this position must have an active LPN/RN license and willing to become a Certified Diabetes Educator.
