Immediate Opening:  Case Management Position:

Independent Living Specialist (ILS) will be responsible for providing services to individuals of all ages with disabilities.  ILS will develop Independent Living Plans and provide training to individual.  ILS supports individuals to accomplish daily living activities both in the home and in the community.  Duties include but are not limited to assisting individual in identifying and accomplishing goals by providing skills based training, phone calls, paperwork, resources and community activities.  Assist individuals with independence promoting skills, teaching activities in areas such as safety, housing, cleaning, cooking, organization, assertiveness, financial management, benefits management, self-care, communication skills, interpersonal skills, self-care, community integration and mobility.  ILS will locate and access community resources, assist with financial planning for limited income, advocate for/with individual and complete required documentation.
