Children with Special Health Care Needs Case Manager – Barton County accepting applications for Case Manager for Health Department Office until position filled.  Part time position working 20 hours per week.  Requires a thorough knowledge of Special Health Care needs in Children as well as the entire life span with special concentration in regards to public health.  Includes visiting clients in their homes to evaluate the social needs of the individuals and of the family unit.  Bachelors’ degree in Social Work or Nursing, with working knowledge of pregnancy and the family unit.  Understanding on effects of illness on a family a plus.  Valid KS driver's license.
