Norton County Hospital has a full time opening for a Case Manager.   This position will be primarily Monday through Friday but may vary with patient activities.  Candidate must have a current Kansas LPN or RN license and they must have or obtain a Social Service Designee certification as well as an Activities Director certification within three months of hire. 

Primary duties include but not limited to the following:

• Utilization Review including patient rounding in the mornings
• Discharge Planning
• Swing Bed and Intermediate Care coordination
• Social Services Designee and Activities Director
• Policy management
• Attend regular trainings
